Program
The Microline PENTAX instrument Repair and Exchange
program guarantees 48 hour or less "In-Plant" turnaround for all
instruments returned for repair. Microline PENTAX objective is to
furnish our customers rapid replacement of returned product with “like-new” manufacturing
plant refurbished equivalent product. Upon receipt of the returned
instrument, Microline PENTAX will ship a Microline PENTAX manufactured
rebuilt equivalent unit. At the discretion of the customer, they
may specify that the returned instrument be repaired and returned,
otherwise, the factory will ship an equivalent factory rebuilt instrument
from Microline PENTAX inventory.
In the event that a hospital requires an immediate
replacement instrument, Microline PENTAX also offers immediate
shipment of a replacement before receiving returned
instrument. In order to ship product we require a hospital and
distributor hard copy of the Purchase Order. There will be a charge
at full cost until the instruments are returned to and received
by Microline PENTAX.

Ordering replacement product using
the Repair
and Exchange Program is fast and easy.
To obtain replacement product simply follow these 3 easy steps:
- Contact Microline PENTAX Repair and Exchange Department (rga@microlinepentax.com)
to obtain Returned Goods Authorization Number (RGA).
- Sterilize and package all products to be returned together with Documentation of Decontamination.
- Ensure RGA Number is written and clearly visible on the outside
of package so that Microline PENTAX can properly acknowledge receipt
of the returned instrument.
Microline PENTAX RENEW™ PRODUCT CONVERSION UPGRADE PROGRAM
With the introduction of the ReNew™ IV product line, Microline PENTAX no longer repairs or replaces the ReNew™ II line. Please contact Microline PENTAX or your distributor regarding pricing on "upgrades" and our repair and exchange program.
Return Policy:
Returns must be made within 30 days of shipment
and must be in original, unopened packaging. Disposable products
must be returned in complete, unopened cases. Returned product is
subject to a 25% restocking charge based on the original purchase
price.
Prior to returning any instrument for repair, contact
Microline PENTAX Repair and Exchange Department to obtain a Returned
Goods Authorization number (RGA). The Repair and Exchange Department
can be contacted by email (rga@microlinepentax.com)
or by calling 978-922-9810. Unauthorized returns will not be accepted.
All returns must be shipped freight prepaid. The instruments will
be returned unrepaired to the sender if the following conditions
are not met:
- Proper cleaning and sterilization after last procedure together
with a Declaration of Decontamination.
- The instrument must be assigned an RGA number.
- The RGA number must be clearly visible on the outside of the
box in which it was shipped in.
Warranty Information:
Three Year Limited Warranty
and Repair Program for all Microline PENTAX Handpieces:
- Microline PENTAX warrants that its instruments are free from
any defects in both material and workmanship.
- Microline PENTAX shall not be held liable for any incidental
or consequential damage of any kind.
- All Microline PENTAX instruments are covered by a three year
limited warranty, valid only to the original purchaser of the
instruments.
- Irrelevant of the nature of the repair, Microline PENTAX will
return to the customer a "like new" instrument completely
refurbished and upgraded to Microline PENTAX factory specifications.

***Gross
abuse or neglect of a Microline PENTAX instrument will void this
warranty.***
***Work performed
on an instrument by anyone other than Microline PENTAX will void
this warranty and will subject the customer to purchase a new instrument
at full cost. ***
Warranty for all Microline PENTAX
Tips, Cartridges, and Accessories:
All Microline PENTAX Tips, Cartridges, and Accessories
are unconditionally guaranteed against defects in material and workmanship.
Microline PENTAX will, at its option and without charge, either repair
or replace any tip which Microline PENTAX determines to be defective
in material or workmanship when used for its intended surgical purpose.
